EMERGENCY: Pet owner will be contacted first in case of any emergency to determine course of action. If pet owner cannot be reached, an attempt will be made to contact the emergency contact listed above to determine course of action. If vet care is needed, we will transport your pet immedicately to receive care. Vet fees are the responsibility of the pet owner.

I hereby authorize the attending veterinarian to treat any of my pets and I accept full responsibility for all fees and charges incurred in the treatment of any of my pets.

The Pet Sitter is authorized to transport my pet(s) to and from the veterinary clinic for treatment or to request "onsite" treatment if deemed necessary. If I cannot be reached in case of an emergency, the Pet Sitter shall act on my behalf to authorize any treatment excluding euthanasia.
