The individual identified below, (hereafter known as "Client") hereby requests and agrees to receive consulting services from Marsha Genin d/b/a Gedalia Healing Arts LLC. ("Gedalia" Client has clearly requested these services from Gedalia. Client agrees to be completely truthful about her current health practices, medications and health history., and agrees to take full responsibility for the choices she makes and the herbs/supplements that may be recommended as a result of her consult with Gedalia. Client hereby releases and holds harmless Gedalia from all liabilities known or unknown and from claims of any kind arising from her consultation and the services performed and the supplements/herbs recommended, that she, or her heirs, personal representatives, executors, successors and assigns now has or may hereafter have against Gedalia. Client hereby irrevocably covenants to refrain from, directly or indirectly, asserting any claim or demand, or commencing, instituting or causing to be commenced, any proceeding of any kind against Gedalia. If any provision of this Consent Form is held invalid or unenforceable by any court of competent jurisdiction, the other provisions of this Consent Form will remain in full force and effect. Any provision of this Consent Form held invalid or unenforceable only in part or degree will remain in full force and effect to the extent not held invalid or unenforceable.

Client has acknowledged that she has reviewed the Services and agrees to the Fees on gedaliahealingarts.com. Client agrees to pay for services at the end of each session. Client understands that several sessions may be needed, and that Gedalia and Client will agree upon the estimated number of sessions needed during the first session.
