I request and authorize the doctors employed at Madison Dental to perform a routine examination, perform a routine cleaning, and provide dental treatment. I further request and authorize the taking of dental x-rays as may be considered necessary by the doctors to diagnose and/or treat dental conditions. I will allow photographs to be taken for diagnostic or educational purposes. I understand that dental treatment includes efforts to guide a patient’s behavior by helping them to understand the treatment in terms appropriate for their comprehension. I will be responsible for any charges incurred for dental treatment.

The exams are also used to help educate people on the best ways to care for their teeth and gums. According to the American Dental Association, x-rays can show tooth decay, fillings, gum disease, and types of tumors. Dental radiographs can also alert your dentist to changes in your hard and soft tissues. In children, radiographs allow the dentist to see how their teeth and jawbones are developing. While X-rays are linked to a slightly increased risk of cancer, there is an extremely low risk of short-term side effects. Exposure to high radiation levels can have a range of effects, such as vomiting, bleeding, fainting, hair loss, and the loss of skin and hair.
