    Please read this agreement carefully before booking and paying for any of our classes, as it forms the legal relationship between us and contains important information about our services and policies.
    If you have any questions or concerns about this agreement, please contact us before booking or paying for any classes. Please allow for any pictures of your child while in yoga.  If you do not want pictures please indicate on form. In the box for allergies in BOLD WRITING above.


    Your Health

    We are not medical professionals and cannot advise on whether you and/or your child (ren) should join the classes or not. If you have any concerns at all about your health and undertaking the classes, you should consult your doctor or other medical professional.
    You should notify us at the outset of any medical conditions that you and/or your child (ren) have or any concerns that you have about participating in the classes. If we have concerns about your and/or your child(ren)'s health and/or ability to participate in the classes, we may require you to leave the class or provide a medical clearance from your doctor.
    You agree that you and/or your child (ren) are voluntarily participating in the classes with the risk that there may be personal injury or illness arising from such participation. You agree to assume full responsibility for any and all damages, which may incur through participation and to release and waive any claims that you have now or may have hereafter against us, our instructors, or the premises at which the classes are held.


    Payment and Cancellation


    If you need to cancel  you must notify us by email at least 30 days prior. 
    We reserve the right to cancel or reschedule any class due to unforeseen circumstances, such as instructor illness, weather conditions, or low enrollment. If we do so, we will notify you by email credit for a future class. If a class is canceled because of low enrollment, the remaining classes that were paid will be reinbursed. If paying by Paypal, there will be a 10% service charge.  If there is a NSF the parent on this form will be responsible for the amount due and bank charges that are applied. If this isn't taken care of within 15 days extra fee of 10% will be added to original amount due for classes.

    If paying by check please make check payable to Toni Boggie. All checks should be given to the front desk employees in a envelope addressed to Toni Boggie/ Yoga.

    Your other option of payment is through my website Toni Boggie choosingjoyyoga.net.

    If paying by website, submit all payments 1st day of every month. By signing this aggreement you agree to a automatic withdrawl from your account.
