This waiver must be completed by each member who will be participating with a band competing at the Stone Mountain Highland Games.

Please fill in the Participant Information and Band Affiliation, attest whether the participant is a minor, then sign the waiver. Participants less than 18 years must have the waiver signed by their parent or legal guardian.
