1st Annual SCS PTO 3 V 3 Spring Basketball Tournament Registration

Fill out this form to register for the event or turn in a printed form to the SCS front office. Registration Deadline: March 15th
This tournament is open to all students in grades 5-8 and community members 18+ (parents, caregivers, staff, alumni, etc.)

Team Divisions will be separated as follows: Girls 5-8, Boys 5-6, Boys 7-8, Adult Coed (18+)

Have a Child or Sibling in Grades K-4? Join us for a FREE Halftime Exibition from 12-1pm exclusively for grades K - 4 to participate with fun games, music and special guests!

**IF YOU DO NOT HAVE A TEAM BUT WOULD LIKE TO PLAY, DO NOT WORRY! FILL OUT THE FORM BELOW WITH YOUR INFORMATION WE WILL PLACE YOU ON A TEAM IN THE APPROPRIATE DIVISION.

Once registration is closed and brackets have been finalized, teams will be informed of the final schedule including team check in times and starting game time, all event details and important info for the day!

TEAM ENTRY FEE: $15 per team player (you must submit payment for each team member included on this form no later than Fri, March 15 to be confirmed as registered)

How to submit payment:

1. Bring a cash or check made out to SCS PTO to the SCS front office in a sealed envelope with your Team Name and Player Names.

2. Pay via venmo or paypal (YOU MUST WRITE "BASKETBALL" AND "YOUR TEAM NAME" IN NOTES SECTION WHEN SUBMITTING PAYMENT)

Team fee includes lunch and one entry into event raffle for each team member, plus prizes for winning teams.

**All proceeds from the event will go directly to the SCS PTO to support school initiatives, PE programs and classroom funding.
