Thankyou for choosing to apply and become a member of the Cook Islands Community WA Incorporated (CICWAI or the Association).

Your support and contribution as a community in WA is greatly appreciated to keep our organisation going.

Membership is effective from the date it is paid in full and entitles you for a calendar year (1 January to 31 December each year) or part thereof.

As of January 1st for every new year, memberships registered in the previous year are expired and a renewal of membership is required. 

Annual General Meetings review membership costs and structure and are generally conducted between Oct-Dec. 

An updated membership application form must be completed so the Association can keep your details current to notify of upcoming events, meetings and any general information. We strongly encourage members to register as early as possible to avoid missing out on any decisive outcomes pertaining to the association. 

Your membership fees support and subsidise some of our CICWAI events, activities and meetings our organisation offers and to support our objectives in its Rules. Please see below an outline of how fees can be allocated on an annual basis.

As a member with voting rights (unless you are a registered minor with a limited registration and therefore no rights to vote), you can also become elected to the Management Committee.

The Committee of Management or it's abbreviated term, COM are responsible in managing the affairs of the Association.

PARENT CONSENT AND RELEASE FORM

I, the undersigned  am the parent or legal guardian of the child/youth named above  who was born as stated and disclosed above and resides also stated above with the disclosed Contact Phone No.

For any situation, I assure that I will be always available at practices, events and can be contacted at all times.

As a parent or legal guardian, I affirm that I have been completely informed all the sport/cultural activities that the child/youth will participate. I understand the general structure of the sport/cultural activities/programs and do not need to be informed of every activity.

I hereby voluntarily release, forever discharge the community, the corporation, its officers, directors, employees, volunteer, and agents from any and all claims, demands, or causes of action, which are connected with my child's participation in the programs or the use of the equipment and facilities.

I agree to pay for all medical expenses incurred and give permission to the doctor or health care professional to provide medical care if necessary.

The information I've given in this form is complete and accurate. 

By signing this form, I confirm that I have fully informed myself of the contents of this Parental Consent and Release Form by reading it before I signed it. 

I warrant that I possess all the rights, powers, and privileges of a parent or legal guardian necessary to execute this document with binding legal effect. I also understand this registration covers participation to all events with no rights to voting in committee meetings and/or toward powers of the Association.

Any notifications such as meetings, events, general information will be sent to the registered email provided by you below. The request for duplicate information allows for a more seamless automation of data. We appreciate your understanding as we work to improve how data is collected and organised.

Terms and Conditions of Membership

1. I/We agree to be bound by the Rules of the Association which can be requested for inspection at any General Meeting. When notification of General Meetings are given, please notify should you wish to review the Rules, understanding there are conditional requirements for any extract of information from the Rules. 

2. I/We agree to the use of any photos, video or audio recordings for the purposes of publishing materials related to the activities of the Association without acknowledgment and without being entitled to remuneration or compensation. The photos/recording may appear in newsletter, community Facebook, electronic or video media and may be available to a global audience. I acknowledge ownership of any photos/recording will be retained by the Cook Islands Community WA Incorporated. I agree that if I wish to withdraw this authorisation, it will be my responsibility to inform the Secretary via email admin@cookislandswa.com
