University of Greenwich  Image Release Form for Minors
Image release form to be completed by a parent/carer/guardian (over the age of 18) for the University of Greenwich on behalf of a minor (person under the age of 18).  

In completing this form, you consent to the capture of:
- Still images (photographs etc.)
- Moving images (film/video etc.)
- Audio recordings
  
  The University of Greenwich is committed to processing information in accordance with Data Protection legislation. The personal data collected on this form will be held securely in our Digital Asset Management system while photo is in use. All photos will be taken on university equipment and never on personal devices.

Commissioning information 
- Content will be used by: University of Greenwich, whose address for the purpose of this agreement is Old Royal Naval College, Park Row, London, SE10 9LS 
- Purpose of the content: Promotion, publicity, and marketing collateral  
- Distribution channels: Print, website, educational and promotional third-party publication or websites, digital and printed displays, the university’s official social media platforms, internal communications, and/or by agents authorised by the university. The image or recording can also be used for instructional or educational use, to include normal teaching and research activities. 
- Distribution countries: Throughout the world, and not just in the United Kingdom, where UK law applies.
- Production contacts: content@gre.ac.uk 
- Copyright: The image and/or recording is governed by the university’s intellectual property policy, as it appears on the university’s website.
