Submit this order form to request access to the Entering Student, Graduating Student, and Alumni Questionnaires. Only ONE form will need to be filled out to gain access to any of these instruments for the academic year. ATS Student Data Services will send a PDF report and raw data spreadsheet for every submitted batch with a minimum of 2 participants. To all that apply, combined reports will be sent. For past-year replacement reports, hard copies of current reports, or custom multiyear reports, please contact Qmail@ats.edu instead.

Participating member schools within a Report Group with eligible participation will receive the same confidential sub-report when any Total School Profile is published. ATS retains the discretion to initiate or terminate a Report Group without prior notice.
