Please designate a primary coordinator that will require access to the Questionnaire Management Interface (QMI). This will be the main point of contact and will be the person to receive the final reports.
If you have a secondary person who needs access to the QMI system and to announcements/updates regarding data collection, please enter their information below.
*For the Alumni Questionnaire please indicate the year the instrument will be administered.
ATS Report Group Inclusion
ATS configures Report Groups based on certain factors over time (e.g., school configuration, affiliation, regional location, religious tradition). Participating member schools within a Report Group with eligible participation will receive the same confidential sub-report when any Total School Profile is published. ATS retains the discretion to initiate or terminate a Report Group without prior notice.
Your school can OPT OUT for the year by selecting the checkbox below (follow instructions that appear). Otherwise, by leaving the checkbox blank, your school is consenting to inclusion in any ATS-defined Report Group.

By clicking the checkbox below, I confirm that the school has reviewed the available QMI resources (viz.
