Angel Scholarship Fund Referral Form

All families are encouraged to contribute to the Angel Scholarship Fund and submit Angel Scholarship Fund Referrals. By completing the form below, you’ll receive entries to win $2,500 toward tuition, fees, and/or your lunch account!

Who counts as a referral?
Anyone with Ohio state tax liability can count as your referral: Mom, Dad, Grandma, Grandpa, Godparent, Aunt, Uncle, your coworkers, older siblings… anyone! Even your own contribution can be submitted as a referral, so don’t forget to enter it.

How it works:

For every $750 contributed through a referral, your family earns one entry to win $2,500.
Refer 10 families? You get 10 entries—there’s no limit!
For every $100,000 raised in Angel Scholarship dollars, we will add another winner.
Our goal is for each family to refer 3 additional contributors to the St. Barnabas Angel Scholarship fund, which will help reduce tuition for all students at St. Barnabas for the 2026-2027 school year.
