INFORMED CONSENT(S) FOR IV NUTRITIONAL THERAPY:
This document is intended to serve as my informed consent to receive IV Nutritional Therapy and/or IM Injection as requested by me to the “provider” Doctor's IV Wellness Clinic.
IV NUTRITIONAL THERAPY INFORMED CONSENT:
I understand that:
1.The procedure involves inserting a needle into a vein and injecting an intra-venous solution.
2. Alternatives to intravenous therapy are oral supplementation and / or dietary and lifestyle changes.
3. Risks of intravenous therapy include but not limited to:
a. Occasionally to commonly:
I. Discomfort, bruising pain at the site of injection.
II. General feeling of warmth during and after injection.
b. Rarely:
I. inflammation of the vein used for injection, phlebitis, metabolic disturbances, and injury.
II. Reactive Hypotension (or rapid drop in blood pressure)
III. Reactive Hypoglycemia (or rapid drop in blood sugar)
c. Extremely Rarely:
I. Severe allergic reaction, anaphylaxis, infection, cardiac arrest and death.
4. Benefits of intravenous therapy include:
4.1. Injectables are not affected by stomach, or intestinal absorption problems.
4.2. Total amount of infusion is available to the tissues almost immediately.
4.3. Nutrients are forced into cells by means of a high concentration gradient.
4.4. Higher doses of nutrients can be given when possible, by mouth without intestinal irritation.
5. The Procedure: The IV intravenous procedure involves inserting a needle into your vein and infusing over a determined period of time, prescribed nutrients (vitamins, minerals, amino acids). Your vitals will be measured prior to and after your infusion. Your temperature will also be taken prior to the start of the procedure.
6. What should I look for after the therapy? Monitor the insertion site for signs and symptoms of infection (redness, swelling, discharge). Notify the provider immediately. If your experience a sustained fever greater than 101, do not delay treatment and go to the ER as this can be a sign of infection. If you experience a serious side effect while you are at home, you should contact the provider, otherwise contact the medical provider or call 911. Some side effects are normal while receiving nutritional therapy, as cells detoxy you may experience some nausea or fatigue. IM INJECTION INFORMED CONSENT:
I consent to an IM Injection; I understand that I may be sore or experience mild pain at the site of the injection. I understand that elevation and the use of a cold pack will alleviate any symptoms. Depending on the supplement that is applied I may experience constipation, heartburn or diarrhea.
LAB TESTS SAMPLE COLLECTION INFORMED CONSENT:
I consent to have a sample of my blood and have the sample submitted to a third-party affiliated laboratory for analysis. To do this a sample of my blood may be taken that are like samples that are normally taken for testing when you are sick. You may also ask to take more samples in the future. These samples may include blood, serum or plasma.
When blood is taken you may feel a slight sting or “pinch” in your arm. You might also get a small bruise from the needle. Rarely, some people faint. There is also a very small risk of an infection any time that blood is taken.
I Consent to have a sample of my blood drawn and have such samples sent to a third-party affiliated laboratory. This consent applies only to the collection sample, I will be signing another consent before any treatment is started. This consent outlined above is: entirely voluntary and I have not been offered any inducement to consent. I have not been given any guarantees or assurances regarding the effectiveness of the treatment.

Statement of Person Giving Informed Consent:
I have read this consent form and understand the information contained in it. I understand the risks and benefits and have had the opportunity to have all my questions answered to my satisfaction. I am aware that other unforeseeable complications could occur. I do not expect the provider(s) to anticipate and or explain all risk and possible complications. I rely on the provider(s) to exercise judgment during the course of treatment with regards to my procedure.
