Maddy's Cat Rescue- Adoption Application Form

Thank you for considering one of our felines!

Please read the following information in detail and answer all questions with as much detail and as honest as possible.

When applying please remember you are doing just that. Applying does not guarantee you will get a cat. It is in our intention to make the best possible placement for our kitties. We reserve the right to deny/ignore/discard any application we see fit. If your application is not filled out completely it will be denied. We do not use a first-come, first-serve process when adopting out our kitties. Instead we focus on best fit for that individual!

Our mission is to place all our felines in loving, permanent homes! Many of these felines were given a second chance by coming to Maddy's Cat Rescue and each have their own unique story!

The adoption process is as follows:

1. You will be contacted by Maddy via email if approved to adopt!

We strive to contact you as soon as possible but please give us time to get through all the forms! Maddy is very busy, not only with the rescue but as well as everyday life! If you have not heard from us in 1 week of submission please reach out via Email, maddyscatrescue@gmail.com

2. Meet & Greet with Maddy and available felines!

3. Complete Adoption agreement & process adoption fee!

4. Coordinate adoption day/pick up! (May be same day as meet & greet!)

All of our felines at MaddysCatRescue are

Spayed and Neutered

FeLv/FIV tested

Vaccinated

Dewormed/treated for fleas and ticks

-You will only be able to meet with currently available felines at your meet & greet! Currently available felines are posted on our Facebook page in an album titled "Currently Available for Adoption"!

-Adoption fees are considered donations and are non-refundable.

-A typical adoption fee is $75! Adoption fee may vary for felines adopted in pairs! Our felines are fully vetted. This fee goes back into us helping more felines!

** YOU MUST BE 21+ TO APPLY FOR A CAT FROM MADDY'S CAT RESCUE

** There is a $10 Non Refundable application fee that will go towards your adoption fee at the end of this application due to the overwhelming issues we have had with individuals filling out application that are either "not ready", not serious, flat out not answer us when we reach out for a meet & greet or simply just no call/no show on the day we set up a meet & greet.

We hope this does not deter you from filling out our adoption application, however, we have to make sure our adoptee's are serious, and ready to adopt from us.

If you do not have a regular veterinarian and have animals, please list the reasoning as to why you do not have a veterinarian for your animals. This is a huge part of owning animals and something we take into serious consideration when approving/denying your application

If you do not have a regular veterinarian, do you know of clinics in your area taking new clients/patients? Your pets should have yearly exams and vaccinations to make sure they are healthy and that requires you to be established with a veterinarian clinic!

Do we have permission to call your veterinarians office and ask for your current pet’s history?

Maddy's Cat Rescue is extremely against declawing, do you understand the health complications that declawing your feline can cause? Do you understand our felines are to never be declawed?

Have you heard of single kitten syndrome?

Why are you looking to adopt a feline?

What personality are you looking for in a feline?

Where do you plan on keeping your feline?

Do you understand that a cat can live up to 20+ years and that you are responsible for them for their entire lifetime?

Are you familiar with Emergency Veterinary Services in your area in case of emergency and your primary Veterinary can not fit you in?

Most emergency veterinarians require payment upfront of $700-$1200 just as initial assessment and diagnostics when lift threatening situations occur. While we know not many have this on hand, we ask that you prepare for this as emergencies do happen
