Instructions:

Please complete all items below. You may save your progress and return to the form at any time. You will receive an email notification once you have successfully submitted the application.
Qualifications
- The applicant must be graduating from Trout Lake High School.
- The applicant should have a grade point average of 2.5 or greater. However, all applications will be considered.
- The applicant must be planning to attend an accredited college, university, or vocational training program no later than October 1st of the year following the award of the scholarship.
- The applicant must include a current (as of your scholarship submission date) transcript of grades and credits earned with the scholarship application. The submitted transcript should include your entire high school education and the first semester of the applicant’s senior year.
Due Date: April 30, 2025 (Late or Incomplete Applications May be Rejected)
