I, [Parent/Guardian's Name], as the parent or legal guardian of the child named [Child's Name], hereby acknowledge and agree to the following terms and conditions regarding the participation of my child in slime play activities offered by Kawaii Shop Manila, hereinafter referred to as the "Service Provider."
1 Responsibility for Safe and Responsible Play: I acknowledge that it is my responsibility to ensure that my child engages in safe, fun, and responsible play while participating in slime-related activities. I understand that my active supervision and guidance are essential to ensure my child's safety.
2 Acknowledgment of Risks: The Service Provider has informed me of the potential risks associated with slime play, and I fully understand these risks. I am aware of the following potential risks and safety measures to be observed while my child is playing with slime
a. Accidental Consumption or Ingestion: I understand that there is a risk of accidental consumption of slime or ingestion of charms by any individual. I will ensure that my child does not put slime in their mouth, and I will closely monitor their play to prevent ingestion.
b. Staining of Clothing or Carpets: I acknowledge that slime may stain clothing or carpets upon contact. I will take appropriate precautions to prevent damage to clothing or property.
c. Allergic Reactions: I understand that some individuals may experience allergic reactions when in contact with slime-containing borax or scents. I will inform the Service Provider of any known allergies my child may have, and I will monitor my child for any signs of allergic reactions during the activity.
3 Safety Rules: I will ensure that my child follows the safety rules provided by the Service Provider, which may include, but are not limited to, the following:
a. No Touching of Face Rule: My child will not touch their face while playing with slime to prevent accidental contact with eyes, nose, or mouth.
b. No Putting of Slime on the Mouth: My child will not put slime in their mouth at any time during the activity.
c. Hand Sanitisation: My child will sanitize their hands thoroughly after playing with slime to prevent any potential contamination.
4 Liability Release: I understand that the Service Provider shall not be held liable for any damages, injuries, or losses arising from my child's participation in slime play activities, including but not limited to those mentioned in section 2 of this waiver.
By signing below, I acknowledge that I have read and understood this Slime Play Consent Waiver and agree to be bound by its terms and conditions. I understand the importance of actively supervising my child during slime play activities and ensuring their safety.
