PE Tennis Equipment Refresh Request

Please submit the following form to request equipment to supplement and/or replace PE tennis equipment you received when you first signed your School Partnership Agreement with the United States Tennis Association (USTA).
Before submitting your form, please ensure your PE class is registered for the current school year at https://www.usta.com/en/home/myaccount/schools.html. Contact Kristin Webb at NWASchools@arktennis.com for help with that and/or completing this form. We would also request that you share a photo of your tennis unit set-up that we can use in social media to help promote our program.
