    We are so pleased to invite your student to the Enchanted Forest Winter Formal, hosted by Angie Greenwalt, Charlotte Cushman and Courtney Fugate.  Students may invite a friend or date.

    Their guest must have your permission and their friend's parent's permission to come (separate waiver will be sent to their family). We reserve the right to decline registration to potential guests

    The dance will be held at Praise Assembly of Beaufort's Fellowship Hall on February 22th from 6:30-10:00. It will be chaperoned. We will provide games, light refreshments, dancing and fun! 

    Please read each of the following sections thoroughly prior to submitting your registration. Each student must submit their own registration. Their guests will have separate waivers emailed to them. The registration is not complete until all the required information has been filled out, parents have signed all forms, and your payment is received. Two students can submit a registration together to get the $25 price, but all waivers must be filled out before registration is complete.

    Once your ticket has been purchased, there will be no refund. If you are unable to attend, you may transfer your ticket by contacting Charlotte Cushman at charlottecushman@gmail.com.

    By registering, you are agreeing to allow your child’s photo to be taken and possibly used on social media. Once we have received your registration and payment, you will receive an email with your ticket for the dance. 

     

  • This is a formal winter dance, so ladies and gentlemen, arrive dressed to impress!  Your wardrobe can be as exquisite as you want. Please use discretion when selecting your attire.

    If you are uncertain about your attire selection, please text me at 843-592-1502.

    The guidelines below are non-negotiable.

    Young Men:  Dress pants, collared shirt and tie or bow-tie and dress shoes. Jacket is encouraged but optional.

    NO jeans, t-shirts, shorts or sneakers.

    Young Ladies:  Formal below the knee gowns or formal pantsuits. 
    NO two-piece with visible midriff, plunging necklines, or plunging backless attire.  

    PLEASE NOTE:  When you submit your registration, you are agreeing to follow these dress code guidelines and the consequences of being sent home without a refund if not adhered to.  There will be NO exceptions.

    We expect all guests to honor each other and the Lord. In order for everyone to have a safe and fun environment, here are the guidelines for the behavior that is expected: No unruly, dangerous and or disrespectful behavior. No profanity. No alcohol, drugs or tobacco of any kind. No firearms or weapons of any kind. No illegal items.

    I have read the dress code and behavior policies for the 2025 Winter Formal and give my student permission to attend.

    I give my permission for my student to invite or bring the guest listed on this registration to the dance. 

    I also understand that certain behavioral guidelines have been placed upon my youth/child during this event. I understand that he/she is expected to adhere to these guidelines. In addition, I understand that failure to follow these guidelines could result in him/her being sent home without refund.

    Medical Release

    During the 2025 Winter Formal Dance: A Night Under the Stars, in case of accident, I do not hold any of the adult chaperones or Praise Assembly responsible for injuries. I give permission for the adult chaperones to authorize medical treatment for my child at the nearest emergency care facility, should an injury or illness occur. Please make sure that any student with allergies has necessary medicine with them. Please make sure that emergency contact number is good for us to reach you.

    WAIVER/RELEASE FOR INJURIES OR HARM


    In consideration of being allowed to participate in the 2025 Winter Formal, I acknowledge, and agree to the following:

    Participation includes possible exposure to and illness from infectious diseases. I am aware of the existence of the risk on my physical person by attending this event; and, I am fully and personally responsible for my own safety and actions during this event.  While particular rules and personal discipline may reduce this risk, the risk of contracting a serious illness and death does exist; and,
    I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, I, HEREBY RELEASE AND HOLD HARMLESS, Charlotte Cushman, Angie Greenwalt and Courtney Fugate, (along with all chaperones and participants) WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

    By submitting your registration, you are agreeing to the conditions and requirements of the dress code, behavior policy and this release form.
