This form is for the submission of Official Transcripts and supporting documents. Please complete all fields and upload the document(s) that have been requested.

Official Transcript Requirements:
- Student name, address and date of birth
- School name, address and telephone number
- Clearly printed grades, subject details and grade/academic year the class was taken in from Grade 9 onwards.
- Signed/stamped and dated by a school official
- Must be in original language
- If the original language is not English the documents must be accompanied by an official translation
