I authorize the St. Albert Minor Hockey Association to conduct any necessary investigation to verify my credentials, qualifications and character to meet the coaching requirements. Should I be selected, I further agree to abide by the Constitutions, Bylaws, and Policies of the SAMHA, Hockey Alberta, and Hockey Canada. I understand that I will not be eligible to coach until a valid VSC (Vulnerable Sector Check) is received, and the required courses are completed. I have read and agree to abide by the Association Coach Code of Conduct (found on samha.ca) and will support the Association's commitment to supporting a positive hockey culture.
