I/We hereby give permission to allow my/our student to participate in the summer camp trip/activity referenced above. In consideration of the Waldorf School on the Roaring Fork (“WSRF”) accepting the student for this summer camp/activity, the undersigned student and I/we agree as follows:
1. The trip/activity may take place away from WSRF property and may involve activities beyond the scope of traditional school functions conducted on WSRF property. In addition, transportation may be provided by private vehicle. We expressly acknowledge, understand, and agree that, in the case of transportation by private vehicle, the insurance carried by the primary vehicle’s owner is the primary insurance coverage.
2. We acknowledge that the WSRF does not have any medical/dental/hospitalization insurance covering students for injuries incurred at school or while on field trip activities. The student has no medical or physical conditions that could interfere with the student’s safety or the safety of others in or during the activity. Student is physically fit and we know of no reason why student should not participate in the activity. The student bears all costs of injury or damage to student. If you have not already done so, you should investigate and must obtain medical insurance coverage for the student. If the student is participating in an overnight or out-of-state field trip, a photocopy of your insurance policy information must be provided to the WSRF prior to the date of the field trip.
3. We acknowledge that student must wear appropriate attire and safety equipment required for the field trip/activity as set forth above, unless otherwise approved by WSRF.
4. I understand that all application fees must be current in order for my child to attend camp at the School. I further understand that ALL forms, medications and records required by the WSRF or the State of Colorado are to be turned in to the School in advance of the above-identified date(s).
5. I understand that in signing this Camp Contract, I am agreeing to accept the policies, rules and regulations of the School as stated in the Parent Handbook, and elsewhere, and as modified from time to time. I understand that the School reserves the right, in its sole judgment, to dismiss either temporarily or permanently any student when such action is deemed necessary and appropriate and/or deemed in the best interest of the student, class, or family involved.
6. This contract shall be interpreted in accordance with the laws of the State of Colorado.
