This request form is for IHS Members who are trainers and would like to have access to their trainee's metrics on the Distance Learning Course and/or ILE Test Prep. Please fill out the form below with you and your trainee's information to have your accounts linked. This form can be completed for just one trainer/trainee or for multiple trainer(s)/trainee(s). Please note that the trainee(s) must be enrolled in the Distance Learning Course and/or have an active subscription to ILE Test Prep before the trainer can receive access.
