    CAMP POLICIES ACKNOWLEDGEMENT AGREEMENT

    1. Payments must be paid in full one-week prior to the week of attendance. Payments are due on the Monday prior. You can pay by filling out the authorized credit card sheet, check or Automatic Funds Transfer, cash and online (currently unavailable).

    2. Deposit: There is a $25.00 deposit fee, per week, after 1st week of enrollment is paid in full. Payment in full is due one week prior to the week of attendance. If payment is not received by one-week prior (previous Monday), your deposit is non-refundable and non-transferable, and you will relinquish your child’s spot.

    3. Enrollment: is contingent upon our ability to staff the weeks, meet the minimum age-appropriate enrollment numbers, and the completion of all registration paperwork, including payment in full for each week.

    4. Cancellation: will not be permitted during the one-week sessions nor refunds. A medical exception may be made on a per case basis. If we cancel the week, you will receive a credit/refund.

    5. Late Pick-Up: There is a $1 per minute charge for picking up your child (ren) past their scheduled time, due prior to continued enrollment in following weeks.

    6. Transfer/Refunds: No transfer policy. Camps will be based on a 1st come and 1st serve policy. Refunds, there will be no refunds unless the City must cancel because we cannot staff, or do not get the enrollment minimums. In which case a full refund will be given.

    7. Electronics: No electronics of any kind are allowed in our Camp Programs. Exception: A child’s cell phone must stay in their backpack.

    8. Attendance Policy: If your child is absent one or more days of Camp during a week that you have made payment, you will not receive a refund. We do not allow make-up days or give credits for missed camp days.

    9. Children with Special Needs: All children are welcome, but your child cannot require one-on-one attention. Prior to enrollment, you need to contact the Recreation Coordinator or Recreation Manager to discuss the appropriateness of the child’s placement. See the parent handbook for more information.

    10. Potty Training: All children enrolled in camp MUST be fully potty trained and able to use the restroom on their own without assistance. If a child has an accident or has difficulties using the restroom on their own, he or she will be removed from camp enrollment and parent, or guardian will be asked to pick up their child immediately.

    11. This is a Recreation program as defined by the State which is exempt from mandated childcare licensing requirements.

I understand programs may be cancelled if minimum required enrollment of 6 children is not met. I acknowledge that I have read and understand the terms set above by the City of Soledad.   • CONSENT FOR EMERGENCY MEDICAL TREATMENT: In the event of emergency accident or illness, program staff will attempt to contact the parent/guardian, or a person designated by the parent, to care for the child and arrange for necessary medical care. If the parent/guardian or person designated by the parent /guardian is not available, I authorize the City of Soledad Youth Program to request assistance from the 911 emergency services and consent to any emergency treatment which is necessary to preserve life, limb, or well- being of my dependent. It is understood that I will be responsible for all costs involved in treatment of this dependent.

  • In consideration of my minor child/ward* being allowed to participate in the Soledad Parks and Recreation Camp program and related events and activities, I, the parent/guardian, acknowledge, understand, and agree that:

    1. I am fully aware and hereby acknowledge that the risk of serious injury from this activity, including, but not limited to, the risk of serious personal and physical or emotional injury, paralysis, death, or other harmful consequences which could arise from my
      child’s/ward’s participation in this activity.

    2. Being fully informed as to these risks and in consideration of SPR allowing my child/ward to participate in the camp program, I
      expressly agree and promise to accept and assume all risks associated with this activity. My child’s/ward’s participation in this activity is purely voluntary and I elect to allow his/her participation despite these risks.

    3. On behalf of myself, my child/ward, my heirs and assigns, I hereby voluntarily waive, release, forever discharge, and agree to indemnify and hold harmless The City of Soledad, its elected officials, officers, agents, employees, instructors, representatives and volunteers from and against any and all liability for personal injury or property damage which I or my child/ ward may have, or which may hereafter accrue to me or my child as a result of participation in any of the activities or events conducted by, on the premises or, or for the benefit of the camp recreation program. I also hereby agree to indemnify and hold harmless The City of Soledad, its elected officials, officers, agents, employees, instructors, representatives, and
      volunteers from and against any and all claims, damages, losses and expenses, including attorney’s fees, which they may incur as a result of my child’s/ward’s participation in the said activities.

    4. The waiver and indemnification I have agreed to as set forth above shall be applicable to all activities of the camp summer recreation program and additionally those activities as set forth below in which I have authorized and requested that my child/ ward be allowed to participate as indicated by placing my initials next to each description of such activity for which I grant my permission.

I authorize and give my consent to the City of Soledad, its employees, agents, or volunteers to photograph/video my child/ward or myself and without limitation, to use such photographs /video in connection with promoting/advertising the programs of the City of Soledad without consideration of any kind.

  • Children who are suspended or whose illness prohibits participation must be picked up from camp within 60 minutes of notification. *Soledad Parks and Recreation reserves the right to modify, repeat, accelerate steps or not use the outlined processes as the situation demands.
