Please explain in as much detail as possible so we know your specific situation. This will help us prepare. WE ARE NOT HERE TO JUDGE ANYONE'S SPECIFIC / PERSONAL SITUATION. WE ARE HERE TO ENSURE YOU HAVE A SAFE PLACE TO BRING YOUR PET, WHICH MEANS WE NEED TO KNOW ALL ABOUT THEM AND THEIR SITUATION.
What days of the week work best for intake appointments / pick ups / drop offs? You will be contacted for a final appointment time and day. (If already arranged, select the last option!.)
Do you understand that once your pet(s) pass our temperament testing, they will be placed up for adoption and may end up with another family at any time? We cannot always guarantee where your pet(s) may end up once they leave our rattery. We can only try our best to stay as updated as we can with all of our new rat owners.
At the time of surrender, is / are the pet(s) in question (to your knowledge) in decent health / physical shape?
If applicable, list any health concerns, conditions, or problems that are known. If not, leave blank. (This includes physical and / or mental ailments or handicaps that are currently known.)
Are there any personality quirks / traits we need to know about your pet(s) before you surrender them to us? Anything helps. Are they shy with new people? Do they fight other rats? Do they sleep a lot and love cuddles? Do they do well with others? Are they aggressive? Put it here!
This is how we get to know your pet(s). Please be as detailed as you possibly can be! If you think it will help us, it probably will! Even if you have a less than friendly pet, we will do our best to help them and rehabilitate them.
I understand that by filling out and signing this form below, that I am relinquishing all ownership of the animal(s) listed on this form. By doing so, I cut all ties with said animal(s) and cannot demand their return if decisions are changed. Individual would have to go through the adoption process to regain ownership of the animal once more.
