HPB - EO Account Issue

Before proceeding, please READ and ACKNOWLEDGE:

**WRONG SELECTION WILL DELAY THE PROCESS OF RESTORING YOUR EO!**

*TIVO ISSUE*
If you received an email informing of TIVO Deactivation, please CREATE a NEW EO Account using your EXISTING email address.

If you do not have an EO account, proceed to select 'Create a NEW EO' and provide your details.

If you have an EO account,

Did Alpha Fitness create your EO account for you?

If no, please contact the vendor which created your account to reset it for you. Alpha Fitness cannot reset EO accounts that were not created by us. If you cannot remember which vendor created your EO account for you, proceed to select 'Create a NEW EO' and provide a fresh email address. DO NOT REUSE THE SAME EMAIL ADDRESS.

If yes, is your EO account disabled for more than 21 days?
- If no, proceed to select 'Reset my EO account' and provide your details.
- If yes, select 'Create a NEW EO' and provide your details.

If you have a WORKING EO account BUT Alpha Fitness cannot tag you, proceed to select 'Multi-tagging to Alpha Fitness' and provide your details.
