I hereby consent to the above named person participating in the program offered by Sokol Minnesota. I recognize that potentially severe injuries, including sprains, broken bones, and permanent paralysis or death, can occur in any activity involving height or motion, including gymnastics. I understand and accept that risk. I also realize that my child will be performing and training for all gymnastics events on various training devices. I further understand that while the payment of tuition and registration fees constitutes a part of the consideration due to Sokol Minnesota for allowing my child to use the facilities and equipment at Sokol Minnesota, an additional and important part of the consideration due to Sokol Minnesota is this signed release form. Therefore, in consideration for allowing my child to use Sokol Minnesota’s equipment and facilities, I hereby forever release Sokol Minnesota, its owners, officers, teachers, and coaches from all liabilities for any and all damages and injuries suffered by my child while under the instruction, supervision, or control of Sokol Minnesota, its officers, teachers, or coaches. As the parent or legal guardian of the aforementioned person, I hereby agree to individually protect for the possible future medical expenses which may be incurred by my child as a result of any injury sustained while training at, for, or under the direction of Sokol Minnesota. This acknowledgement of risk and waiver of liability, having been read thoroughly and understood completely, is signed voluntarily as to its content and intent. I, the parent or guardian of the above named person, give permission for emergency medical treatment of my child if I cannot first be contacted.

Photo Release: Occasionally Sokol Minnesota has a need to use photos of its participants in print or other media. These may be to showcase the program in our Czech and Slovak Sokol Minnesota newsletter, the Slovo, in feature articles about Sokol Minnesota programs such as in the Highland Villager, West Seventh Community Reporter, Pioneer Press, etc. and in publicity about the gymnastics program, which may include program brochures and posting performance photos or videos on the Sokol Minnesota Facebook page. Please know that we use discretion when using photos, and we do not typically publicize names. In order that we may respect your position on the use of photos, please review, indicate, and sign below.

Signature of Adult or Parent/Guardian. Parent/Guardian Expectations: Be your child’s biggest supporter. Submit medical waivers and class registration documents promptly. Submit class fees by the second week of each session. Alternate payment plans can be set up on a case-by-case basis, but need must be communicated by the second week of the session. Try to be on time, and if you are going to be late picking up your child, please call so we can notify your child. Please try to arrive no more than 5-10 minutes before practice and pick up your child within 5-10 minutes after practice. Inform us if someone besides you will be picking up your child. Gymnasts must remain in the building until parent/guardian has come inside to get them. If you have a concern, please bring it up to us as soon as possible in person or by email. Encourage gymnasts to inform instructors when they are not feeling well or have an injury. We can make efforts to modify class activities to accommodate each gymnast. Parents and guardians are welcome to watch practice. We ask that you refrain from talking or motioning to your gymnast during practice. We need the gymnasts focused and attentive during practice, and any distractions will affect the quality of the training and can even lead to injury. If a gymnast is absent for five consecutive classes without notification, a student on the waiting list will be offered the spot in class. Gymnast Expectations: No jewelry or baggy clothing. Stay off the equipment before class. No gum or food. Look both ways before crossing the gym. Ask to leave class (to use the restroom, get a drink, or get picked up early). No shoes or socks. Only one person on the equipment at a time. Do not try new skills without a coach (both at the gym and at home). Respect all classmates, coaches, and equipment. Never say “I can’t” – always try our best! By signing below, I acknowledge that I have read and will comply with Sokol Minnesota Gymnastics expectations.
