As the parent or legal guardian of the above named player, I am familiar with my son or daughter’s wishes to participate in your sports program. I am aware that with participation in sports programs comes the risk of injury to my child and that this increases in contact sports such as football and lacrosse. I have had the opportunity to understand the risk inherent in such athletics. In addition, I am aware that participation in sports activities will sometimes involve travel to or from an activity or event. In connection with the above-named player’s participation in such athletics, I and such player assume all the risks and hazards incidental to the conduct of such activities and the travel involved including, but not limited to, such risks and hazards associated with COVID-19, and hereby release, indemnify, and hold harmless Port Washington Youth Activities (PYA), its officers, directors, coaches, members, consultants, supervisors, managers, employees, sponsors and any person involved in such athletics with the above-named player or transporting such player to or from any activity scheduled or arranged by PYA. I also agree to conduct myself (and to advise the above-named player to conduct theirself) properly and in accordance with the PYA code of conduct with the understanding that any improper conduct on my part (or their part) may jeopardize the above-named player’s participation. I consent to allow PYA take photos of the above-named player for marketing purposes. I consent to any emergency medical treatment of the above-named player and hereby assume responsibility for payment for such treatment.
I further acknowledge my obligation to honestly and accurately respond to any health screening assessment conducted by PYA and to update the PYA Executive Director and/or his designee in the event that my own, or any member of the above-named player’s household Covid-19 Health situation changes during participation in sports activities with PYA.
PYA PARENTS CODE OF CONDUCT
Parents must read, understand and sign this form prior to their child participating in our program. Any parent exhibiting improper conduct at any game or practice will be asked to leave and be suspended from the following game or practice. Repeat violations may cause a multiple game suspension and further action by the PYA Disciplinary Board.
I agree:
I will not force my child to participate in sports.
I will remember that children participate to have fun and that the game is for youth, not adults.
I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice or other sporting event.
I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting; refusing to shake hands; or using profane language or gestures.
I will never ridicule or yell at my child or other participants for making a mistake or losing a competition.
I will encourage my child to play by the rules.
I will emphasize skill development and practices and how they benefit my child over winning.
I will respect the officials and coaches and their authority during games and I will never question, discuss, or confront coaches and/or officials at the game location; to the extent that I need to speak with any coaches, I will do so at an agreed upon time and place.
I will demand a sports environment for my child that is free from drugs, tobacco and alcohol and I will refrain from their use at all sports events.
I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches of the team, or have been asked to do so by an official coach and have completed necessary background paperwork.
I will refrain from entering dugouts or field sidelines/benches deemed as areas for coaches and players during games and practices, unless I am one of the official coaches of the team.
