By registering your child, you agree that Limelight Dance Center and its contractors, have the right to use any forms of media including but not limited to video, photographs, and other promotional materials indefinitely. COVID 19 RELEASE- COVID 19 has been declared a worldwide pandemic by the World Health Organization. COVID 19 is extremely contagious, Limelight Dance Center can’t guarantee that anyone at the studio will not become infected with the COVID 19 virus. I/We understand that there is a risk of becoming exposed or infected by the virus during any in person event. By attending, viewing, participating we hold harmless Limelight Dance Center from all claims and liabilities both known and unknown. By registering, attending, viewing, or participating in a Limelight Dance Center event or entering the venue you acknowledge full responsibility and forever release and discharge Limelight Dance Center, and any of their affiliates, directors, employees, agents, contractors, volunteers, and venues from any and all claims, suits, actions, and liabilities arising from or relating to injuries, illnesses, accidents, or damages from participating or viewing this event. If injury or illness occur, you understand that Limelight Dance Center, and any of their affiliates are not financially responsible and will not offer any reimbursement or compensation. By registering, attending, viewing or participating in a Limelight Dance Center event or entering this venue, you release all liability and agree with all policies set LDC. I agree to allow mt dancer to be filmed and photographed. Media may be used for advertisements. By registering, attending, viewing, or participating it is agreed that your own insurance policy will be the only source of any reimbursement in the event of injury, illness, or any other instance. By attending, viewing, participating we hold harmless LDC from all claims and liabilities both known and unknown. As the parent, you are attesting that you have read and agreed to all LDC policies. I am signing this waiver on behalf of the studios entire dance team and their attendees. I am guaranteeing that not only I agree with the policies. All parties have been informed of the policies prior to attending. All parties have agreed to hold LDC harmless in the event of injury, illness, or other instance. All participants, viewing guests and all attendees have been made aware of the LDC waiver. By attending all parties have agreed to the above-mentioned conditions and policies.
