Emergency Medical Authorization Purpose: To enable parents and guardians to authorize the provision of emergency medical treatment for children who become ill or injured while at baseball activities when parents or guardians cannot be reached. In the event reasonable attempts to contact me at the above numbers have been unsuccessful, I hereby give my consent to transfer of the child to nearest hospital or any hospital reasonably accessible. This authorization does not cover major surgery unless the medical opinions of two other licensed physicians or dentists concur the necessity for such surgery are obtained prior to the performance of surgery. This also releases WYBSO from charges applied as a result of transferring your child to a doctor/dentist/hospital not covered by your insurance company. Facts concerning the child’s medical history including allergies, medications being taken, and physical impairments which a physician should be alerted.

Sudden Cardiac Arrest and Lindsay's Law Consent: What is Lindsay’s Law? Lindsay’s Law is about Sudden Cardiac Arrest (SCA) in youth athletes. It covers all athletes 19 years or younger who practice for or compete in athletic activities. Activities may be organized by a school or youth sports organization. Be sure you have reviewed the Lindsay's Law video and guidelines and consent to have done so. A link to the guidelines and video can be found on our league website at www.neobaseball.org and under the "Info for Parents" tab.

Code of Conduct: All board members and coaches are volunteers. They donate their time, money and knowledge to the players. It is your responsibility to respect the other players, coaches and umpires throughout the season. If you have an issue with a coach or other players, please report the situation to the WYBSO board members. If you choose not to volunteer then you shall not complain, degrade, or confront coaches during practice or games.

Media Release Permission: I, the parent/guardian, hereby consent that the photographs or videos taken of my child during games or practices while they are registered as a player for WYBSO may be used by us on our social media. This permission is with the understanding that neither WYBSO nor its representatives will use any media of my child for any commercial value or receive monetary gain.
