Informed Consent:
You are invited to complete this survey designed to understand the needs and priorities of transgender and gender non-conforming (TGNC) identified persons in Middle Tennessee. This survey is completely optional and anonymous. It will ask you to disclose demographic information and opinions based on your lived experiences. Only the preliminary questions are required, all others are optional. The survey should take you about 10-20 minutes to complete.
As a thank you for your participation, at the end of the survey you will have the option to go to a second encrypted form where you can opt-in for the opportunity to receive a thank you gift ranging from gift cards for local and online vendors to other items from supporting businesses. This same form lets you opt-in to receiving survey results and updates. This is entirely optional.
Purpose:
The purpose of this survey is to connect with the transgender and gender non-conforming population of Middle Tennessee and identify areas of need. The results of this survey will be used by inclusiontn and partner organizations to create programs and resources to best serve the community. Only the findings of the survey will be shared with other organizations, not individual responses. All identifying information will be removed from the data, anonymizing all responses.
Who Can Participate:
Anyone who lives in Middle Tennessee, self-identifies as transgender, gender non-conforming, non-binary, gender expansive, or another associated gender identity besides cisgender, and is over the age of 18 is encouraged to complete this survey.
Additionally, parents, guardians, and trusted adults of transgender and gender non-conforming youth who live in Middle Tennessee are welcome to complete the survey to represent their needs. Please do not answer on behalf of the transgender and gender non-conforming youth, only answer to reflect your needs as a support system, provider, and caregiver.
If you identify as transgender and gender non-conforming and are also a parent, guardian, or trusted adult to TGNC youth, please complete the survey twice. Once for yourself and your needs and a second time to reflect your needs as a support system, provider, and caregiver.
Further Information:
The only risks to the completion of this survey are potential psychological distress due to the survey content and potential loss of confidentiality. If at any time you find yourself uncomfortable, take a break from the survey or cease taking the survey and seek support. Inclusion tennessee will encrypt the responses, remove any identifying information, and will keep the responses inside the organization. Personal information will not be publicly shared.
There are no direct benefits to completing this survey beyond aiding the transgender and gender non-conforming community in Middle Tennessee. The results from the survey will be used by inclusion tennessee to identify the needs of the community and develop programming to best support the community. The anonymized results may also be shared publicly and with other research/community-based organizations.

Preliminary results will be released within a few weeks of the survey closing and a final report will be released after approximately 2 months. If it has exceeded that time frame, please contact us about when it will be released.
