  • Happy Tails Canine Rescue will provide all medical care, heartworm and parasite preventative. Are you prepared to assume the responsibility of feeding, bathing, grooming, and caring for your foster as well as making accommodations for them if you go out of town?
  • Will you follow all veterinarian instructions provided and administer all prescription medications and preventative to your foster?
  • Do you agree that Happy Tails Canine Rescue will make all medical and adoption decisions for our rescue dogs?
  • Will you let us know if you are planning to take your foster out of town or state for family visits or recreations?
  • Though we do not accept vicious dogs into our program, we do not know the histories of many of our rescues. Are you prepared to contact us immediately if your foster dog bites anyone or injures another person or animal?

  • Foster Contract:

    By sending and signing this contract electronically, I acknowledge that I have completely read this application and comprehend it fully. I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Happy Tails Canine Rescue animal fostered by me. I certify that the above information is correct, and I understand that the information will be verified. I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Happy Tails Canine Rescue and it's members from any claims, damages, costs, injuries or actions incurred because of the care or actions of the foster dog. I accept full responsibility for the dog(s) actions at all times, and release Happy Tails Canine Rescue from any liabilities or damages that may be incurred because of fostering such dog(s). I agree to have Happy Tails Canine Rescue complete reference call checks and conduct a home visit inspection to be able to approve my foster application. I understand that Happy Tails Canine Rescue is the rightful owner of my foster pet unless I have an approved adoption application and I have paid the adoption fee.

    I agree to the following:

    foster the dog until adoption, help write a bio for my foster pet and/or give personality desciption to a rescue team member within a week of fostering them for their Petfinder account, send high quality photo(s) and video(s) weekly to the rescue, join the foster family group chat, participate in at least one rescue event while the foster dog is in my care, and post my dog on social media and tag the rescue.

    I understand that a big part of being a foster is advertising and socializing my foster pet, and I am willing to be an active participant in the rescue. 

    If I plan a trip, I will notify the rescue at least a month in advance of my trip. It is my responsibility to take the dog with me, secure a vacation foster (who needs to be approved by the rescue), OR I will make accomodations for my foster dog(s) just as if they were one of my pets at no expense to the rescue.

  • Adoption Contract:

    I understand a dog is a lifelong commitment and a valued member of the family. I commit to giving the dog the best possible life and embracing him or her as part of my family. The dog will live inside with me and my family and will never be confined, tethered, or tied up in a neglectful or abusive manner. I will give the dog the love, medical attention, and care it needs and deserves. The dog will remain on flea and tick and heartworm prevention monthly and receive its annual vaccinations and heartworm check yearly.  I will always keep my dog’s best interest at heart. If I am at any point unable or unwilling to properly care for the needs of the pet I adopted from Happy Tails Canine Rescue, or unable to abide by this contract, I will contact Happy Tails Canine Rescue.

    I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Happy Tails Canine Rescue and it's members from any claims, damages, costs, injuries or actions incurred because of the care or actions of the dog I adopted from them. I accept full responsibility for the dog's actions at all times, and release Happy Tails Canine Rescue from any liabilities or damages that may be incurred because of the dog I adopted from them. 

    It takes a dog three(3) days to decompress, three (3) weeks to learn your routine, and three (3) months for their personality to come out and adjust to their new home. Therefore, we require our adopters to give their new family member AT LEAST 3 weeks before considering returning the dog to Happy Tails Canine Rescue. 

    Note: If you complete a surrender form, or contact the rescue needing/wanting to surrender the dog you adopted, it does not guarantee that the Rescue is able to take the dog back at that time. The rescue will do everything it can, with the resources it has at the time of the request. 

    Note: We do not offer reverse transport. If you have adopted a dog from the rescue and need to surrender the dog back to us, it is your responsilibity to arrange transport of the dog back to the rescue. 

    Upon adoption, the adoption fee must be paid in full through Venmo, PayPal, Cash App, or Stripe. Happy Tails Canine Rescue does not place dogs on hold. All dogs will remain listed as available until the adoption application has been approved and the adoption fee has been received.

    Legal ownership of the dog remains with Happy Tails Canine Rescue until both the adoption fee has been paid in full and the dog has been physically transferred to the adopter.

    Adoption fees vary by dog, with the average range being $400-$500. Fees are determined based on factors including the dog's age, medical needs, and the veterinary care received while in rescue.

    Happy Tails Canine Rescue is a 501(c)(3) nonprofit organization that is entirely volunteer run. All adoption fees are used to support the rescue's mission and are reinvested directly into the care of current and future rescue dogs, including food, supplies, veterinary care, monthly preventatives, and other necessary expenses.

    All adoption fees are non-refundable. Prospective adopters are encouraged to discuss the adoption fee with their rescue representative prior to finalizing the adoption.

    Failure to comply with any terms of this adoption agreement may result in the dog being reclaimed by Happy Tails Canine Rescue.
