This form must be completed by learners or their representative who require a replacement certificate or a copy of their certification data held by LCL Awards. There is a fee of £50.00 inc. vat for the issue of each replacement certificate and for the provision of certification data held by LCL Awards a fee of £30.00 inc. vat. Complete the following details in full and attach (where required) a current passport quality photograph.
