Please fill out this registration form for our Sensei Joey Tolentino Kumite Seminars & In-House Tournament. This is a pre-registration-only event. This form must be completely filled out by the participant and their guardian. One form must be completed by each participant. Registration deadline is on February 23. A $10 late fill will apply for registrations sent in after the 23rd. Registration closes on Tuesday, 2/27~ no registrations will be accepted after this date. Doors open at 8:30 AM. Modified WKF Rules. Required Equipment: Gloves, Shin/Foot Gear, mouthpiece (absolutely mandatory to compete in Kumite), chest protector & headgear for athletes under 13. Groin guard for male athletes.

This Competition is Pre-Registration Only - No registration will be allowed on the day of the tournament.  This form serves as a payment application, payment authorization, registration, waiver of all rights, and agreement.   APPLICATIONS MUST BE FILLED OUT COMPLETELY.

Agreement

I, the undersigned parent, guardian/ participant, herby agree to allow the individual (s) named herein to participate in the aforementioned activity(ies), and I further agree to indemnify and hold harmless Tokon Martial Arts, LLC, all of its officers, employees or officials harmless from and against any and all liability for any accident or injury or any claim of damages which may be suffered by the aforementioned individual(s) arising out of or in any way connected with his/her participating in this activity without regard to the active negligence of Tokon Martial Arts, LLC or its officers, agents, employees or officials. I understand that COVID-19 is an ongoing public health situation.  I understand that COVID-19  (including variations and mutations is highly a highly communicable virus that can cause among other conditions, severe respiratory illness, sometimes leading to death, in individuals who contract it.  I will not attend this event if I am exhibiting symptoms of COVID-19 symptoms which include, without limitation, fever, cough, shortness of breath or difficulty breathing, chills, repeated shaking with chills, muscle pain, headache, sore throat, new loss of test or smell, or if I have been exposed to a third party who’s exhibited COVID-19 symptoms preceding my participation in this activity. Participant expressly waives any and all claims for damages or liabilities that arise from the use of the property or participating in a Tokon Martial Arts, LLC Activity and hereby expressly waives any claim or right to damages to property , personal injury or damages occasioned by the loss of use of the property or cancellation of the Tokon Martial Arts, LLC sponsored activity when the loss or cancellation is due to an interruption in power to the property.  I give Tokon Martial Arts, LLC permission to use photographs of my child/myself taken during this event to be used for publicity purposes on the means of publicity.  I understand that my child/ I may or may not be identified by name when photos are used for publicity purposes.
