CAMP POLICIES
- CLOSED CAMPUS. I understand that for the safety of my child, the campground is off limits to family or friends or anyone not a staff member of Denton Camp. If there is an emergency I will be informed immediately and if I need to contact my child concerning an emergency, I will do so through the Camp Director or Camp Office.
- NO PHONE CALLS. I understand that my child cannot make or receive phone calls while attending camp. No electronics (incl phones) are permitted.
- PRESENT ENTIRE TIME. I understand that my child is expected to be present throughout the event with no personal interruptions (i.e. sports/music practice, vacation, etc.) so that staff can best minister to my child.
- DISREGARD OF RULES. I understand that if my child cannot stay at camp due to extreme disregard for camp rules/violation of state laws, I will be required to retrieve my child with no continuation of camp activities and no refund.
- PRAY. I will hold up staff & children in prayer during camp.
- CANCELLATION. I understand if my child must withdraw registration for any reason, I will be refunded less $75 fee.
RELEASES
Health Information Release. This health history is correct and complete, as far as I know. I hereby authorize Denton Camp's health officials to share health information and health history with the other staff members on a need to know basis. This includes the camp director, program directors, and counselors that have the minor in their care. The purpose of this disclosure is for the necessary staff to be prepared in advance for any medical emergencies. I agree to the release of any records necessary for insurance purposes. The health information that may be disclosed will be from the Health Information Form, Medication Authorization Form and Immunization Records. I authorize release of medical information to Denton Camp's camp practitioner, for necessary treatment while attending camp. I also authorize the release of medical information from my child's PCP office to Denton Camp if necessary. This authorization is valid for the year of the current registration. I understand that I may revoke this authorization at any time by submitting written notice of the withdrawal of my consent. I also understand that this information is released to aid in the treatment and care of my child.
Basic First Aid & Medication Authorization. I hereby give permission to the camp to provide basic first aid, and administer prescribed medications as authorized by my child's PCP. I also give permission for Denton Camp to administer camp stocked over-the-counter medications on an "as needed" basis, as indicated on the health form, and as directed by the health supervisor. I give permission to Denton Camp to seek emergency medical treatment including ordering x-rays or routine tests. In the event of an emergency, I give permission to the camp to arrange necessary related transportation for my child. In the event I cannot be reached in an emergency, I hereby give permission to the practitioner selected by the camp to secure and administer treatment, including hospitalization, for my child.
Indemnity Release. I agree that Denton Camp, its agents, officers, employees, trustees and volunteers will not be liable for any injury, death, damage and/or loss to myself or my child, and/or anyone claiming on my or my child's behalf, and I further agree to hold harmless, indemnify and defend Denton Camp, its officers, staff, agents, employees, trustees and volunteers for and from any and all liability, claims, losses, injuries, expenses, fees and/or damages arising out of any injury, illness or death to myself or my child or property damage during my or my child's attendance at Denton Camp. The minor child herein has permission to engage in all camp activities. While Denton Camp has safety protocols in place to manage allergen related issues, I understand that a minor with specific allergies or intolerances has a role and responsibility in the avoidance of the known allergen. I agree to educate my child, who has allergies or intolerances, to ask questions, read labels, or abstain from the substance in question when in doubt.
Media Release. I grant permission to Denton Camp, its agents, and its employees the irrevocable and unrestricted right to produce photographs and video taken of my child, myself, and members of my family while at Denton Camp for any lawful purpose including publication, promotion, illustration, advertising, trade, or historical archive in any manner or in any medium. I hereby release Denton Camp and its legal representatives from liability for any violation or claims relating to said images or video. Furthermore, I grant permission to use the statements of my child, myself, or my family members given during an interview or evaluation with or without my name for the purpose of advertising and publicity without restriction to time limit or geographic area. I waive my right, my child’s rights, and my family’s rights to any and all compensation stemming from the use of these materials.
Participation Agreement. In consideration for the opportunity to participate in camp activities, the Participant (or parent/guardian if the participant is a minor) acknowledges and accepts the risk of injury associated with participation in and transportation to and from the activity. The Participant (or parent/guardian accepts personal financial responsibility for any injury sustained during the activity or during transportation to and from the activity. Further, the Participant (or parent/guardian) promises to indemnify, defend and hold harmless the activity sponsor or its agents, employees, volunteers, or any other representatives (collectively referred to hereinafter as the “Sponsor”) for any injury related directly or indirectly out of the described activity or transportation to and from the activity, whether such injury arises out of the negligence of the Sponsor or otherwise. If a dispute over this agreement or any claim of damages arises, the Participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the Participant (or parent/guardian) and the Sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel of the American Arbitration Association for final resolution. Description of Summer Camp Activities: Activities include, but are not limited to: canoeing, fishing, swimming and water activities, group games, team building activities, and off-site activities.
