Please read and sign once you fully acknowledge and agree to the following terms, then please go ahead to fill in the booking form.

 

Acknowledgement and Waiver

 

By signing this agreement, I acknowledge that all of my questions and concerns that I am about to receive from SIX HARMONY STUDIO have been answered to the fullest extent. I acknowledge that I have been advised of the facts and matters set forth below, and I agree as follows:

• I am not pregnant or nursing. I am not under the influence of alcohol or drugs. I do not have medical or skin conditions such as but not limited to: acne, keloid scarring, eczema, psoriasis, I have not taken blood thinning medication, I do not have HIV, Hepatitis/Jaundiceif, Epilepsy; I am not undergoing chemotherapy, and diabetes. If I do have any skin or medical condition, I understand that I must let Terran.tattooart know beforehand.
• I fully understand that SIX HARMONY STUDIO, when performing a tattoo, does not act in the capacity as a medical professional. The suggestions made by SIX HARMONY STUDIO are just suggestions. They are not to be construed as, or substituted for, advice from a medical professional.
• I understand that a tattoo will be performed using appropriate techniques, instruments, and pigments.
• I agree that the studio does not have a way of identifying if I’m allergic to the elements or ingredients, or processes used during my tattoo, and I agree to accept any and all risks associated with my choice of getting a tattoo.
• I understand that infections can occur due to pigment sensitivities and/or lack of proper hygiene when I choose not to follow proper aftercare.
• To ensure proper healing of my tattoo, I agree to follow the written and verbal aftercare instructions that will be provided by SIX HARMONY STUDIO until healing is complete.
• I understand that a tattoo may take several weeks to heal properly. I understand that I am making a permanent change to my body, and no claims about the possibility of reversing these changes have been made or implied by SIX HARMONY STUDIO.
• I allow my tattoo to be photographed and be used for tattoo studio portfolio showcased.
• I acknowledge that SIX HARMONY STUDIO does not offer refunds.
• I indemnify and hold harmless the tattoo studio against any claims, expenses, damages, and liabilities.
• I confirm that the information I provided in this document is accurate and true.
• I do not have a physical, mental or medical impairment or disability which might affect my well being as a direct or indirect result of my decision to have a tattoo.
• I understand that neither tattoo studio is responsible for the meaning or spelling of the symbol or text that I have provided to them or chosen from the flash (design) sheets.
• I understand that over time, the colors and the clarity of my tattoo will fade due to natural dispersion of pigment under the skin. I also understand that if my skin color is darker, the colors will not appear as bright and vibrant as they do on someone with lighter skin.
• I understand that if I have any skin treatments, laser hair removal, plastic surgery, or other skin-altering procedures, it may result in adverse changes to my tattoo.
• I understand that tattoo inks, dyes, and pigments have not been approved by the federal Food and Drug Administration, and the health consequences of using these products are unknown.
• I acknowledge that SIX HARMONY STUDIO is not responsible for any injuries that may or may not occur during my visit on the premises of SIX HARMONY STUDIO.
• I understand that the tattoo artist (Terra Nguyen) has the authority to refuse anyone for any reason.

Six Harmony Studio Policies

By signing this agreement, I acknowledge and I agree to all the POLICIES at SIX HARMONY STUDIO as follows:

1. Age Policy

• You must be at least 18 years of age with a valid government-issued photo identification

2. Non-Refundable Deposit Policy

• All deposits placed at SIX HARMONY STUDIO private studio are strictly non-refundable and non- transferable. We do not guarantee tattoos due to various healing results from client to client. All services are final sale. Before booking, please be 100% sure you are ready for this commitment because tattoos are a very expensive and long process.
• The artist may inform to discontinue the project if the client do NOT follow or break the aggreements.
• The artist trains herself and enhances her skill day by day to provide you the best quality tattoo service. Therefore, any appointment/session of the same signed up piece/project that booked after 1 year since the first appointment/session will be applied with the current rate of the time. All reschedule inquiries should NOT be further than 3 months, unless it caused from my end. If you really need to do so by all chance, your signed up project will be applied with the current rate of the time. The ideal is getting the project that you signed up done within 1 year.
• No depositing in advance to secure the rate is applied and accepted.

3. Missed or Late Cancellation Policy

• Your deposits will be credited to your last session. Any missed appointments or no-shows without a 48 hour notice will result in the complete forfeiture of your deposit, you must place another deposit to book another appointment. If you let me know before then, I will be more than happy to switch the appointments around for you.

4. Touch-Up Policy

• We guarantee touch ups up to 2 months after your last appointment. You may book your touch up appointment after your last session regardless of the artist(s) checking, if your tattoo healed correctly well, a touch up is unnecessary.
• Touch ups booked after 2 months will be charged at the regular rate. Please make sure to book your touch up appointment before 2 months if needed.
• However, tattoos done on hands/palms/fingers, feet, under ankle, inside ear, elbow are not covered under the touch up policy because they are difficult to take care of and tend to fade quicker than tattoos done on other areas. Therefore, you will be charged the regular rate if touch ups are needed.

5. Design Change Policy

• A draft design will be sent to you before your appointment 3-4 days in advance. The final design will be sent the night before your appointment.
• If the design has already been emailed to you and you decide you want to get something completely different, then I will require another deposit to start that design. This means that if you were getting, for example, a half sleeve of a phoenix but decided you wanted to switch to a dragon, then you would have to put down another deposit. This is because every subject is different and regardless of what you are switching your design to, it will take me time to come up with a drawing that meets the standard before your appointment. And if the tattoo appointment date is right close, this might lead to reschedule your appointment since there is not enough time to get the design ready.

Please note unfortunately I don't work on minors even with parent's consent and parent's ID.

A Government Issued ID (18+) must be checked once you are arrived for your tattoo appointment.

Please prepare reference photos and your desired placement where you want to get the tattoo on, I'll get back to you to collect them via email.

This Agreement will be applied for your future projects as well so you don't have to fill more forms unless we update the form.

 

Please allow up to 7 days to receive a response from Six Harmony Studio via email.

 

If you DO NOT hear back from us in 7 days, please assume Terra cannot accept your appointment at this time due to various reasons (example: Terra does not specialize in this style or booking is closed due to high demand. 

Once you have been contacted regarding booking, we will request for a nonrefundable deposit to be sent to info@sixharmonystudio.com
