Use this form to opt-in to participate in the BNI Utah North 20/20 Success Guarantee Program for new and renewing members. Participants in the program must be accepted for membership with a 2-year membership term, make payment between February 1st, 2024 and December 31st, 2025 and agree to meet the program terms and conditions as outlined below.
