  • By completing this form, I am submitting an appointment request inquiry. It does not guarantee an appointment. In response, if we are a good fit, I will receive an email/text with application status and instructions on how to move forward with an appointment date and time option.

    Generally, new guests are able to be seen within 1 week of appointment submission request, but guests with limited availability may have to wait up to 2 weeks to be seen.

     
    I understand that we have a strict 24 hour cancellation policy and if I do cancel my appointment with less than 24 hours notice, the cancellation fee is 50% of the service(s) total booked. No show/No call the cancellation fee is 100% of the service(s) total booked.
