SPIRITUAL LIFE

At camp, it is important that we be able to share this with the children, as they will probably understand the Gospel on a more personal basis if they hear and see it working in our lives! Be prepared to share this story in person to the staff and also to the campers if the opportunity arises!

2. I give InterAct Ministries of Canada permission to contact persons indicated to obtain and verify information regarding previous ministry experience with children and youth.

3. I give InterAct Ministries of Canada permission to contact persons named as references to ascertain my suitability for working with children and youth.

4. I will initially provide InterAct Ministries of Canada with a current (no later than 3 months old) Criminal Record Check. Going forward with InterAct Ministries, a criminal record check will be required every 2 years.
*We need to receive your criminal record check before we can approve your application.

5. I give consent to InterAct Ministries of Canada/Midway Bible Camp Staff to authorize medical treatment in case of emergency.

6. I agree to respect and abide by Midway Bible Camp's Code of Conduct (available upon request)

7. I also acknowledge Midway Bible Camp (MBC) under InterAct Ministries of Canada will be providing training as a condition of my participation. I understand that I will be trained to appropriately respond to all reasonably anticipated circumstances which may be encountered during participation at Midway Bible Camp, including vulnerable sector training and I agree to abide by InterAct's Child/Youth Protection Policy.

8. I understand that my participation in activities may have undesired and unanticipated consequences. Notwithstanding this I hereby release and forever discharge InterAct Ministries of Canada and MBC and all affiliates, members, directors, officers, leaders, agents, volunteers and employees from any and all actions, causes of action, suits, claims, demands, liabilities, including negligence or breach of any duty of care, and expenses I have now, or may have in future in connection with, arising from or related to my involvement with or participation in this activity.

9. PHOTO RELEASE: I understand photos/videos of me taken during this trip may be used in InterAct Ministries promotional materials and I am not entitled to any compensation or royalties with respect to the use of the photos. I waive any rights for approval or inspection of any photos.
