Private Investigation Agreement

CLIENT requests that CIS, LLC. Initiate Investigation and or surveillance of the Target commencing on teh above listed start date and continue until the minimum budget hours or approved hours are met. CLIENT also requests that CIS, LLC. Initial budget hours Additional hours approved by CLIENT

The pay rate is $125.00 UDS per hour with a 4-hour minimum.

All PAYMENT are NON-Refundable including cancelations. However, the CLIENT can reschedule for a later date with proper notice, or CIS, LLC will offer credit towards your next assignment.

CLIENT shall PAY $500.00 (Five Hundred US Dollars), in the possession to CIS, LLC. to serve as an initial retainer to specifically commence investigation on the date selected above. Should the retainer be insufficient or more hours are requested and approved by the CLIENT (See additional form), CLIENT agrees to promptly pay CIS , LLC the additional amount in full upon receipt of an invoice (Quickbooks Direct Pay Link). If the CLIENT interferes with the investigation in any way, the investigation shall cease, and all monies paid shall be forfeited to the company. CIS, LLC. agrees to notify CLIENT when 80% of the retainer as posted is exhausted. CIS, LLC. will cease from any further investigation once the retainer has been depleted. CLIENT shall promptly pay CIS, LLC. any additional funds CIS, LLC. deems necessary to continue the investigation at CLIENT’S request. CIS, LLC. agrees not to exceed the authorized budget and retainer without the CLIENT’S authorization (See additional form) unless the additional expenses were incurred in the process of following the Subject and/or obtaining information critical to the purpose of this investigation. Any investigation where the CLIENT wants. CIS, LLC to start within 48 hours shall be classified as a RUSH assignment and additional fee of $250 USD will be applied to the retainer shall be non-refundable in exchange for CIS, LLC. placing pending cases on hold to start the CLIENT’S case within the 48-hour period. The hourly rate for surveillance / investigation is billed at $125.00 per hour per Investigator with a 4-hour minimum.  All investigated actions/ accepts shall be deducted from the investigation clock including but not limited to Telephone and/or Personal Interviews, Inquiries, Background, On-Line Investigations, Internet, Computer Database Research, Business/Corporate Record Searches, Report Writing, milage and or travel time (unless client aggress to pay $.75 per mile) (See additional form) and CLIENT conferences. CIS, LLC. will forward CLIENT statements (update via crosstrax) and/or invoices listing in detail any and all time and expenses incurred in the investigation. In many instances the CLIENT may need CIS, LLC. to be available on a “stand-by” basis. In such circumstances, the CLIENT shall be charged a minimum of $300.00 per day per Investigator. CLIENT agrees to compensate CIS, LLC. at the agreed upon rate for any court appearances or depositions prior to or subsequent to the completion of the investigation, whether under order of subpoena or not and regardless of the party requesting the appearance. If courtroom testimony or deposition regarding this matter or any matter related to this investigation is required, the fee will be $125.00 per hour per Investigator plus expenses with a 4-hour minimum per Investigator per day will be imposed. Any appearance over 4 hours will incur a flat fee of $750.00 per Investigator per day. All courtroom and deposition fees are due in advance. A retainer for the amount must be received 7 days prior to scheduling the Investigator(s) for court or deposition. CIS, LLC. shall not be responsible for court delays or cancellations. Forty-eight (48) hours’ notice is required in the event of any cancellation.

The Investigation Clock is the amount of time purchased by the CLIENT and is billed at $125.00 USD per hour. All hours MUST be paid for prior to the start of any work. If additional hours are needed CIS, LLC will send the CLIENT an approval form and invoice. For special situations and CIS, LLC is nearing the end of the approved investigation clock it is up to the discretion of the field investigator to continue or halt the investigation depending on the current situation if CIS, LLC Management and or CLIENT is not available for approval.

CLIENT recognizes that CIS, LLC. will likely incur expenses directly associated with conducting said investigation. As such, CLIENT shall be responsible for all reasonable expenses, including but not limited to, mileage (.75 per mile), tolls, OR THE TRAVEL TIME CAN BE COUNTED AGAINST THE INVESTIGATION CLOCK, parking, overnight accommodations, meals*, admission fees, videotapes, CD’s, DVD’s, photographic film and processing, long distance/roaming/calling card/collect calling phone charges, public/private transportation, confidential source fees, or any unforeseen expenses necessary to acquire information as requested by CLIENT. Mileage shall be invoiced at the rate of .75 per mile starting and ending at the CIS OFFICE for cases more than 60 MILES OUTSIDE OF THE WEST LA AREA. *Daily per-diem for food on overnight assignments is $100.00 per Investigator if agreed upon prior to scheduling the work

ALL PAYMENT ARE NON-REFUNDABLE.

The CLIENT is responsible for all the appropriate use of the information gathered by CIS, LLC and takes sole liability for any civil / criminal damages that may be incurred by TARGET if the information is misused in any way. CIS, LLC will restrict certain information obtained through the course of the investigation if were is prohibited by LAW. CIS, LLC DOES NOT GAURENTEE a result. We only report the facts as they arise. And if CIS, LLC has a reasonable belief that if the information may cause physical injury and or death of the TARGET. CIS, LLC WILL NOT RELEASE THE INFORMATION unless so ordered by a court.

All CLIENTS will receive their updates by email via crosstrax. You will be emailed a username and password. Please check it regularly for all your updates. Investigators may call the client In situations where immediate approval or notification is needed

CLIENT agrees that CIS, LLC., at CFRA, LLC sole discretion, may use subcontracted Investigators and may immediately engage one or more additional Investigator(s) at the agreed upon rate and billed in addition to the hourly Investigator rate (within the authorized budget) which CLIENT hereby AGREES to pay in the course of any assignment where CIS, LLC. determines that one or more additional Investigator(s) is needed immediately, such as when a separate Investigator is needed to initiate an undercover inquiry, or the Subject of our investigation leads CIS, LLC. to an unusually difficult setting, such as downtown office buildings, airport, public amusement / entertainment / sporting events, etc.
