ACKNOWLEDGMENT OF RISK: The undersigned acknowledges there are inherent risks associated with equine activities such as described below and hereby expressly assumes all risks associated with participating in such activities. The inherent risks include, but are not limited to, the tendency of equines to behave in ways such as, running, bucking, biting, kicking, shying, stumbling, rearing, falling, or stepping on, that may result in an injury, harm or death to persons on or around them; the unpredictability of equine's reaction to such things as sounds, sudden movement and unfamiliar objects, persons or other animals; certain hazards such as surface and subsurface conditions; collisions with other animals; the limited availability of emergency medical care; and the potential of a participant to act in a negligent manner that may contribute to injury to the participant or others, including failing to maintain control over the animal or not acting within participant's ability.
ASSUMPTION OF RISK - RELEASE OF LIABILITY: In consideration for the services provided by North Star Equestrian, its agents or employees, Nicole Packer and Walt Packer, I hereby voluntarily assume all risks and danger including injury, harm, or damage to my horse, the horse I am riding or leasing, my property, my child or myself from any cause whatsoever. This includes, but is not limited to, the use of the facilities, horses, and services of North Star Equestrian, and of its agents or employees. I further release and discharge North Star Equestrian, any of its agents or employees, Nicole Packer and Walt Packer, from all liability, claims, causes of action, harm, injury or damage of any kind that may happen to my horse, the horse I am riding or leasing, my property, my child or myself arising directly or indirectly from activities or services provided. This release is binding on myself as well as my heirs, assigns, personal representatives and successors.
DISCLOSURE OF RIDING ABILITY: As owner, rider, parent, or guardian, I acknowledge that I have disclosed to North Star Equestrian, its agents or employees, my riding ability and that of my child(ren) named hereinafter, and all facts pertinent to the behavior, temperament and abilities of my horse(s). This includes restrictions you, your child, or your horse might have and any other pertinent information that could inhibit the students ability.
