*Once you have submitted your payment there are NO REFUNDS*.
*This serves as your Vendor Acknowledgment. By signing below AND submitting your payment you are stating that you UNDERSTAND and AGREE with all the terms & conditions outlined on this form*.
*Vendors are expected to be responsible for how well their booths do. It has been proven that no matter how well attended (or lack thereof) an event is- the vendors who are KNOWLEDGEABLE & PASSIONATE about their products/services AND engage attendees with enthusiasm DO WELL!
*This is a rain or shine engagement*.
*The Midwest Regional Black Culture Fest and/or it's partners reserve the right to cancel the event for reasons beyond their control*
*There is NO sharing booths with other businesses. Each business present is responsible for the full vendor fee.*
*It is required that ALL vendors be set up at least a half hour BEFORE the events begin*.
*Parking and loading in/out is covered by the vendor* We do our best to make sure we have volunteers to assist.
*Any loss/damage of your items during transportation or the duration of the event is not covered by the Midwest Black Culture Fest and/or its partners*
Consent to Photograph /Videotape / Interview Individuals- By signing below you also agree to being photographed/recorded at the event for promotional purposes:
*Terms & Conditions- I hereby agree and give my permission for Midwest Black Culture Fest and/or partners to record, film, photograph, audiotape or videotape myself (hereinafter collectively referred to as “Works”) and to display, publish or distribute these Works for the purpose of publishing, posting on the Midwest Black Culture Fest or it's partners' website and posting on social media sites. I hereby waive any right to approve the use of these Works now or in the future, whether the use is known to me or unknown, and I waive any right to any royalties related to the use of these Works. I understand that the Works may appear in electronic form on the internet or in other publications outside of the Midwest Regional Black Culture Fest or its partners' control. I agree that I will not hold the Midwest Regional Black Culture Fest or its partners responsible for any harm that may arise from such unauthorized reproduction.
