Before filling our your BOI Report, please collect the following information from each beneficial owner. For more information on who is a Beneficial Owner, click here:

Reporting Company
- Full legal name
- Any trade name or "doing business as" (DBA) name, if applicable
- Complete current U.S. address
- State, Tribal, or foreign jurisdiction of formation
- For a foreign reporting company only: State or Tribal jurisdiction of first registration.
- Internal Revenue Service (IRS) Taxpayer Identification Number (TIN) (including an Employer Identification Number (EIN)

  - U.S. passport
  - State driver's license

Company Applicants

A Company Applicant is the individual who directly filed the creation or first registration document for the company with the secretary of state or similar office. Many times, this individual is the same person as one of the Beneficial Owners for the Reporting Company, but could be another individual (e.g. an attorney, accountant, etc.) who filed information for the creation of the company.

A Company Applicant must be an individual, and not a company or legal entity.
