Photo and Video Consent Form

It is important for CASK Research to have a stock of photos of children with CASK-related disorders to use on the official website, pamphlets, flyers and ad hoc social media. With this in mind, do you agree to allowing the photos shared to be used for multiple uses?

I declare that:

I agree to CASK Research using in print and digital versions for promotional purposes the image(s) and or video(s) supplied. I understand the image(s) and or video(s) will be accessible to the public from CASK Research’s website. I understand the image(s) will be freely, publicly available for download from CASK Research's website and may be disseminated via other channels (e.g. our social media channels and leaflets) and potentially also shared by third parties. Identities will not be published unless additional consent is given. 

In giving this consent, I understand and declare that:

I am giving this consent in my own free will, and not under duress or in any form of threat;

That the images containing the subject in this consent shall be held by the organization in accordance and compliance with the GDPR guidelines (the General Data Protection Regulation);

That my information herein, as well as the images to be shared to the recipient shall not be disclosed to any other party without my written consent;

That I hereby allow the use of images where the Subject is included shall belong to the organisation for their own disposition;

I hereby agree to waive my rights to any claims to whatever the organization may use of the image;

 

By submitting this form with my signature below, I am affirming to the provisions mentioned above and the effectivity of this agreement shall commence.
