CALL-OFFS FROM RETURNING STAFF WERE DUE BY END OF DAY FRIDAY, AUGUST 29!

ANY NEW STAFF MEMBERS THAT JOINED THE TEAM AFTER THIS DATE CAN FILL THIS OUT. IF YOU WERE NOT GIVEN A SET DEADLINE, NO WORRIES! JUST SUBMIT YOUR TIME-OFF REQUEST AT LEAST TWO WEEKS IN ADVANCE. IF YOU ARE UNABLE TO DO SO, JUST EXPLAIN WHY AT THE BOTTOM OF THIS FORM. THANKS!
PLEASE NOTE: Only a set number of staff can request off each night to account for last-minute call-offs due to illness or emergencies. If your request isn’t approved, please don’t take it personally—we do our best to be fair and give everyone equal opportunities for time off.
