Important Information

This form must be completed and submitted to FFNC in advance of you being appointed to any sanctioned fixtures in 2026.

FFNC will not rely on details that were submitted in any previous season/year.

There are four sections to complete:

• Statement by a Supplier Declaration
• Match Official Playing, Coaching and General Appointments Information

The details listed below will be used by Football Far North Coast in Schedula to make payments to match officials.

This means a junior match official can nominate the bank details of a parent/guardian, however, it will be the name of the registered match official that is shown in Schedula.

It is critical that you advise FFNC if you make any changes to your nominated bank account.

Payments to match officials are ordinarily made on a fortnightly basis and typically credited on a Friday.

SECTION 3 - STATEMENT BY A SUPPLIER DECLARATION

Under the Pay As You Go (PAYG) legislation and guidelines produced by the Australian Taxation Office, I provide you with this written statement that, for the supply I am making (as a match official) and further supplies of this type that I make to you;

• The supply is made to you in my capacity as an individual and the supply is made in the course of an activity that is a private recreational pursuit or hobby.
• Therefore, I am not quoting an ABN. You should not withhold an amount from the payment/s you make to me for the supply. I agree to advise you in writing if circumstances change to the extent that this statement become invalid.

I wish to acknowledge the statement given in relation to PAYG legislation and hereby authorise Football Far North Coast to credit my match payments or reimbursements to the financial institution account listed above 

(Parent/Guardian to sign on behalf of match officials under 18 years of age).

SECTION 4 - MATCH OFFICIAL PLAYING, COACHING AND GENERAL APPOINTMENTS INFORMATION

NOTE that each individual must ensure that their availability to officiate is nominated/updated accurately in Schedula not later than 6pm Sunday each week.

Further to availability (non-availability) that you nominate in Schedula, please communicate by email to referees@ffnc.net.au or text to 0434 856 992 not later than 6pm Sunday each week if there is any specific information relevant to potential appointments.

FFNC works through the appointments process each Monday/Tuesday with appointments published at 5:30pm each Tuesday.
