Annual Application for Use of ECIA Dog Park
In submitting this application, resident agrees to abide by all Eldorado Community Improvement Association Dog Park rules and to assume personal responsibility and liability for any damage done to individuals, to dogs, and/or to property and further agree to hold harmless the ECIA from any liability.

Requirements for Annual Dog Tag:
Documentation for current rabies vaccination (Distemper, Bordetella, and Parvovirus are strongly suggested as well.)
Photo of dog (you can bring one or we can take the photo)
Must be a Member in Good Standing with ECIA
