Multimedia and medical release
I hereby authorize and grant permission to The Mobile Lip Experience LLC and those acting under their direction to: (a) record my child's participation and appearance on video tape, audio tape, film, photograph or other medium; and (b) exhibit, copy, publish or distribute such recording in whole or in part without restrictions or limitations for any educational or promotional purpose which they deem appropriate. I understand that I nor my child will receive any compensation for the use of such recordings.
I understand that in consideration of my child being given the opportunity to participate in the program, I voluntarily release and hold harmless The Mobile Lip Experience LLC employees, students, and agents from any and all claims, causes, and action, injuries, damages or losses of any kind that may arise from my child's participation. Moreover, I acknowledge that The Mobile Lip Experience LLC does not provide health or accident insurance for my child and I assume full responsibility for all costs and expenses associated with any medical emergencies and doctors' visits due to injuries or illness of any nature.
