Each cat must ONLY be brought in its own humane trap measuring at least 30"L x 9"W x 11"H (NO multiple cats/kittens in a trap). Cats that do not arrive in a humane trap will be turned away on the day of surgery. We do have traps available for rent if needed. (Plastic cat carriers are NOT accepted)

I understand that due to limitations of the TNR program, no other veterinary services will be provided. If a problem is identified by the veterinarian on duty I will be informed of the condition at the time of release and it will be my responsibility to seek veterinary attention for this cat elsewhere.

Any cat deemed severely ill or injured by the veterinarian will be humanely euthanized. Best efforts will be made to contact you, the caregiver, first. If you cannot be reached via phone within a reasonable time, I give permission to Rescue Village to proceed with euthanasia if it is determined the cat has a condition causing release to be deemed inhumane.

While we do not use antibiotics at the time of uncomplicated spay/neuter surgery, we do sometimes find an infection or wound that should be treated with antibiotics. If indicated by physical exam findings and instructed by a veterinarian, I agree to the administration of a single dose of long-acting antibiotics as appropriate. I understand there will be a $25 charge for this medication.

Rescue Village's TNR program only provides services to un-owned, outdoor, community cats. I certify to the best of my knowledge that the cat(s) I am admitting are un-owned and do not have indoor homes. I understand that feral cats face risks during handling, anesthesia, and surgery and I hold Rescue Village, its volunteers, staff, and facilities harmless should a cat experience complications, injury, escape, or death. I also release Rescue Village, its volunteers, staff and facilities from any liability incurred while transporting or caring for these cats:
