All applicants are requested to submit a photo of themselves. The photo must be high resolution. Acceptable photos include a business or theatre headshot, senior photo, or similar. Please no selfies or photos with filters.

By checking this box I am agreeing and granting ARIEL Theatrical and the Heart of ARIEL Scholarship committee permission to use the uploaded photo in any and all of its publications, including the ARIEL website and social media, for use in the promotion of the Heart of ARIEL scholarship.
Use of Scholarship Application Responses

By checking this box, I understand that all or some answers in my scholarship application may be used for promotion of the Heart of ARIEL scholarship, regardless of the outcome of my application. I agree therefore that by submitting my application gives ARIEL Theatrical permission to use it for other purposes. I understand that ARIEL Theatrical will acknowledge my authorship of the application responses should its contents be used.
Video Recording Agreement

By checking this box, I agree to provide a recording of my scholarship application responses if I am awarded the Heart of ARIEL Scholarship. I agree to schedule a time to meet with the committee to record in person, or will agree to provide a high resolution video for promotional purposes.
By signing the space below, I certify that all information is correct and that I am the person completing this application. If awarded the Heart of ARIEL scholarship, I also agree to complete the recording and provide confirmation of enrollment of the college or trade school i will attend during the fall semester. Finally, I understand that if awarded a scholarship this year, I will not be eligible for an award the following year, but I may apply again in 2027.
