This form is to be completed in order to make any changes to your original Childcare Contract with Cabin Kids Childcare.  Once the form has been approved by Michelle Nicholas (Childminder) it will become legally binding and serve as proof that changes have been noted.  As per original contracts, the 4 week notice period will apply.
