Please submit one form for each child. When you are ready to complete the application, please be sure to set aside sufficient time. You will not be able to save the application and return to it later. If you exit the application before completing it, we will not receive it and you will have to start again.

Optional extended hours from 7:30-8:30 and 3:30-5:30 are available on a limited basis and are dependent upon meeting the enrollment minimum for those times. We will do our best to accommodate preferred schedules, but if the minimums are not met, the hours may not be offered. See our website for rates for individual schedules.
