  • INSTRUCTIONS

    All Meet Referee's shall complete a Meet Ref Report within ten (10) days of meet completion.  Fill in all applicable fields relating to the meet.

    A List of Officials who helped at the meet must also be sent with this report.  You can use the WSI Officials Excel Form, a snippet/pdf of the list in OTS, or your own prepared list (it should include the officials first and last name, club, and their LSC if not WSI). 
    Please attach this info in the File Upload section of the report.

    Please preview the report by using the PREVIEW PDF button at the end, to ensure all info is correct. If your meet is more than 7 sessions, please use a separate form for the additional sessions for the meet.

    If you need to stop in the middle of completing the form and leave the site, you can come back to where you left off by clicking the SAVE button. This will send you to a pop up window; click SKIP Create Account at the very bottom to get the link to your partially completed form.

    When you hit SUBMIT at the end, this report will automatically be sent to MIKE MELLOCK, the Wisconsin Swimming Officials Chair.
    You will be emailed a confirmation along with a copy of the report.
